Adenoids are small patches of lymphatic tissue located at the back of the nasal cavity, just above the roof of the mouth. They play a role in the immune system by trapping bacteria and viruses entering through the nose. In childhood, adenoids are usually large because the immune system is developing, but they tend to shrink naturally as children grow older. However, when adenoids become chronically enlarged—a condition known as adenoid hypertrophy—they can lead to a range of problems such as nasal obstruction, mouth breathing, snoring, and even potential effects on overall growth and development, including height.

1. How Adenoid Enlargement Affects Breathing and Sleep

When adenoids are enlarged, they partially or completely block the nasal airway. Children with this condition often resort to breathing through the mouth rather than the nose. Mouth breathing disrupts the normal flow of air and can alter the balance of oxygen and carbon dioxide in the blood.

A major concern is the impact on sleep quality. Adenoid hypertrophy is one of the main causes of obstructive sleep apnea (OSA) in children. OSA leads to repeated pauses in breathing during sleep, resulting in fragmented sleep and reduced oxygen levels (hypoxia). Since deep sleep stages are when most growth hormone (GH) secretion occurs, sleep disruption can significantly interfere with a child’s growth process.

2. The Role of Growth Hormone and Sleep

Human growth hormone is primarily released during deep slow-wave sleep. It is responsible for stimulating the growth of bones, muscles, and tissues. When sleep quality is poor—due to frequent arousals or reduced oxygen saturation caused by enlarged adenoids—the secretion of GH can be diminished. This may lead to slower growth rates and, in some cases, temporary growth failure.

Studies have shown that children with chronic sleep-disordered breathing have lower levels of insulin-like growth factor-1 (IGF-1), which mediates the effects of growth hormone. As a result, these children may appear smaller or shorter than their peers. Once the underlying airway obstruction is corrected—often through adenoidectomy or adenotonsillectomy—growth parameters tend to improve.

Adenoid hypertrophy can also indirectly affect height growth by influencing feeding and nutrition. Children with enlarged adenoids sometimes experience difficulty chewing and swallowing, leading to reduced appetite and inadequate caloric intake. Poor nutritional intake translates into insufficient nutrients for proper growth. Moreover, chronic mouth breathing and low oxygen levels can increase the body’s energy expenditure, diverting calories away from growth processes and toward maintaining basic metabolic functions.

4. Adenoid Facies and Growth Development

Chronic adenoid enlargement often leads to a characteristic facial appearance called *“adenoid facies.”* This includes an open mouth, elongated face, narrowed upper jaw, and sometimes dental malocclusion. These skeletal changes occur due to prolonged mouth breathing, which alters the natural growth pattern of facial bones. While these changes are mostly cosmetic and structural, they indicate that the child’s facial and cranial growth patterns are being affected—showing how persistent airway obstruction can influence skeletal development.

Although adenoid facies mainly involves facial structure rather than overall height, it highlights how chronic obstruction can alter growth processes driven by oxygen supply, muscle function, and bone development.

  •  Koren et al. (1997) reported that children with obstructive sleep apnea caused by adenoids and tonsils were significantly shorter than age-matched controls. After adenoidectomy or adenotonsillectomy, many showed rapid “catch-up growth” in height and weight within six to twelve months.
  •  Bar et al. (1999) observed that serum levels of IGF-1 and IGFBP-3 (growth-related proteins) were lower in children with OSA, but improved after surgical removal of enlarged adenoids and tonsils.
  •  Montgomery-Downs et al. (2005) demonstrated that treating sleep-disordered breathing not only improved sleep quality but also restored normal growth patterns in affected children.

These studies collectively suggest that the effect of adenoid hypertrophy on growth is functional rather than structural—meaning that it interferes with sleep, oxygenation, and hormone secretion, all of which can impair growth temporarily. Once the obstruction is relieved, the child’s body often compensates with accelerated growth.

6. Reversibility and Prognosis

The good news is that growth impairment due to enlarged adenoids is usually reversible. After an adenoidectomy (surgical removal of adenoids), most children show improved appetite, better sleep, and a surge in growth hormone activity. This “catch-up growth” phase typically restores the child’s height to their genetic potential within months to a year, provided no other underlying growth disorder exists.

It is essential, however, to identify and treat adenoid problems early. Prolonged untreated obstruction can have more lasting effects on facial bone growth and dental alignment, which may require orthodontic correction later.

7. Preventive and Supportive Measures

In addition to medical or surgical management, parents can support optimal growth in children with adenoid issues by:
  •  Ensuring adequate nutrition: balanced diet rich in protein, vitamins, and minerals.
  •  Promoting good sleep hygiene: regular sleep schedule and nasal hygiene.
  •  Regular medical checkups: to monitor growth charts and detect early signs of growth delay.
  •  Addressing allergies or infections: since recurrent nasal inflammation can worsen adenoid swelling.
