While we fully recognize the medical necessity of analgesics during pregnancy to avoid or manage more detrimental situations, our findings are significant for clinical guidance regarding self-medication with the OTC medication Paracetamol.
INTRODUCTION
While fever and pain can be incapacitating and pose risks, especially in pregnancy, the use of medication may lead to long-term consequences for the offspring. Although there is a wide range of fever and pain medications available, only a few are deemed safe for use during pregnancy. N-acetyl-p-aminophenol (Paracetamol, Tylenol, APAP) has been the first-choice and widely utilized medication for pain and fever since 1893. Paracetamol is an over-the-counter (OTC) medication sold under various brand names, either in its pure form or in combination with vitamin C or caffeine. Paracetamol is considered to be safe at therapeutic doses and is neither classified as an opioid nor a coagulation inhibitor. However, Paracetamol can cross the placenta easily, thus having a direct effect on the fetus. Several retrospective and cohort studies have explored whether the use of Paracetamol during pregnancy influences pregnancy outcomes or the health of children. Currently, the effects of prenatal Paracetamol on brain development and children’s behavior are under intense discussion. In addition, a cohort study undertaken in Denmark in 2008 found a positive link between prenatal Paracetamol exposure and the prevalence of asthma in children. This was further corroborated by the research of Shaheen et al. , which connected Paracetamol to antioxidant gene polymorphisms and wheezing when consumed in late pregnancy. Recently, Magnus et al. published a significant Child Cohort Study that showed an association between pre- and postnatal exposure to Paracetamol and the development of asthma. A meta-analysis indicated that Paracetamol usage during the first trimester appears to increase the risk of asthma in childhood. Nevertheless, the relationship between Paracetamol and childhood asthma risk differs across studies and lacks clarity regarding causality, such as changes in immune development due to Paracetamol exposure. Strong evidence from mouse studies showed a decrease in fetal liver HSCs, which may be linked to a heightened risk of airway inflammation in the offspring.
The objective of our current research was to identify the Paracetamol intake pattern during pregnancy in a longitudinal study. In addition, we sought to examine potential effects and crucial periods of Paracetamol intake on fetal immune development by evaluating the relationship between Paracetamol and HSC frequencies in umbilical cord blood.
