Introduction

Etiology and Pathogen
For decades, Rhinosporidium seeberi was classified as a fungus. However, molecular studies (particularly rDNA sequencing) have placed it within the Mesomycetozoea group, which lies at the interface between animals and fungi. This group includes fish and amphibian pathogens, suggesting an aquatic origin.

- Eukaryotic microbe
- Obligate aquatic organism
- Cannot be cultured in vitro (a significant barrier in research)
- Forms characteristic sporangia within host tissues
Epidemiology
Rhinosporidiosis occurs most commonly in:
- India: Especially in Tamil Nadu, Kerala, Odisha, Chhattisgarh, and West Bengal.
- Sri Lanka: Endemic and frequently reported.
- Other reports include Africa, South America, and some parts of Southeast Asia.
Risk Factors:

Pathogenesis

When the sporangia rupture, they release endospores that can infect adjacent tissues or distant sites via autoinoculation.

Clinical Features
1. Nasal Rhinosporidiosis (Most Common Form)
- Polypoidal, friable mass in the nasal cavity or nasopharynx
- Single or multiple red, vascular lesions
- Bleeds easily on touch
- Nasal obstruction
- Mucoid or bloody nasal discharge
- Often mistaken for nasal polyps or neoplasms
2. Ocular Rhinosporidiosis
- Affects conjunctiva or lacrimal sac
- Presents with conjunctival mass or epiphora (watering eye)
- Recurrent conjunctival growths
3. Cutaneous Rhinosporidiosis
- Secondary to spread from nasal or ocular sites
- Presents as verrucous, nodular, or ulcerative lesions
- May occur at trauma sites
4. Disseminated Rhinosporidiosis
- Very rare
- Involves skin, lungs, bones, or viscera
- Seen in immunocompromised individuals

Diagnosis
Clinical Diagnosis:
- Based on history of exposure, endemic area, and characteristic lesion appearance.
Microscopic Examination:
- Biopsy of lesion is essential.
- Hematoxylin and eosin (H&E) stain shows large sporangia filled with endospores within granulomatous tissue.
- Mucicarmine and PAS stains can help identify the sporangial wall.
Cytology:
- FNAC (Fine Needle Aspiration Cytology) can demonstrate sporangia in subcutaneous or conjunctival masses.
Molecular Techniques:
- PCR has been used in research settings to confirm diagnosis but is not widely available in clinical practice.
Treatment
1. Surgical Management
Surgery is the mainstay of treatment.
a) Excision of Lesion:
- Complete wide surgical excision with cauterization of the base is standard.
- Cautery helps destroy any residual spores and reduces recurrence risk.
b) Instruments Used:
- Electrocautery
- Diathermy
- Cryotherapy (in selected cases)
c) Challenges:
- Lesions are highly vascular and bleed profusely.
- High recurrence rate due to spillage of endospores during surgery.
- Multiple procedures may be necessary for recurrent cases.
2. Medical Management
No antimicrobial agent has proven to be completely curative. However, some agents have shown partial efficacy or are used as adjuncts to surgery.
a) Dapsone (Diaminodiphenyl Sulfone)
- Most widely used drug in rhinosporidiosis.
- Mechanism: Inhibits maturation of sporangia and accelerates fibrosis.
- Dose: 100 mg orally daily for 3 to 6 months or longer.
- Reduces recurrence and helps in resolving small residual lesions.
Side Effects:
◦ Hemolysis (especially in G6PD-deficient patients)
◦ Methemoglobinemia
◦ Agranulocytosis (rare)
b) Antifungal Agents
- Amphotericin B, ketoconazole, itraconazole have been tried but with limited success.
- Rhinosporidium seeberi is not a true fungus, explaining the poor response.
c) Other Immunomodulators
- Some case reports have used corticosteroids or immunosuppressive agents in disseminated disease with variable results.
3. Supportive and Preventive Measures
- Nasal irrigation with antiseptic solution post-operatively (in selected cases)
- Avoidance of bathing in ponds or rivers in endemic areas
- Early recognition and removal of recurrent lesions
- Education in rural communities to reduce exposure risks
Complications
- Repeated recurrence due to incomplete removal
- Nasal deformities or septal perforation after multiple surgeries
- Conjunctival lesions may cause vision disturbances
- Disseminated disease can affect lungs, bones, and other organs (rare)
Recurrence and Prognosis
- Recurrence rates are high (10–70%) depending on the extent of surgery and presence of residual spores.
- Long-term prognosis is good with complete surgical excision and dapsone therapy.
- Malignant transformation has not been reported.
Future Directions and Research
- Attempts are ongoing to culture Rhinosporidium seeberi to understand its life cycle better.
- Molecular therapies targeting specific stages of sporangial development are under investigation.
- Vaccine development is theoretically possible but not close to clinical application.
